Flu Shot Information And Forms

Due to the high demand for the influenza vaccine this year, we strongly encourage our patients to call in advance to book an appointment. We are still accepting walk ins, however, you may experience a longer wait time than usual. We also recommend you download and print the screening form for the flu shot at home, have it filled as much as you can, and bring it with you to the appointment.

COVID-19 Screening

If you experience any symptoms of COVID-19 ( fever, cough, shortness of breath, etc), have recently traveled outside of USA in the last 14 days or have been in contact with someone who has tested positive for COVID-19, please do not come to the pharmacy. Instead, you may phone the pharmacy and we will provide you with further instructions.
